This form is for Ä¢¹½ÊÓƵAPPstaff members. The purpose of this form is to send a request to Ä¢¹½ÊÓƵAPPLibrary to have a library account set up for you.
Once it has been set up, you will be able to access Ä¢¹½ÊÓƵAPPLibrary services and our licensed online databases using your MyÄ¢¹½ÊÓƵAPPlogin. If you don’t have a MyÄ¢¹½ÊÓƵAPPlogin, you will need to contact the IT Help Desk.
Once you have a MyAu login and you submit the form, a library staff member will contact you.
Note: The personal information collected on this form will be used for the purpose of processing your request for the assistance from Ä¢¹½ÊÓƵAPPLibrary. This personal information is collected under the authority of the Post Secondary Learning Act that mandates the programs and services offered by Athabasca University, and will be protected under the privacy provisions of Alberta's Protection of Privacy Act (POPA)
